I declare that the information above is true and correct to the best of my knowledge. 

I understand that it is the sole responsibility of individual trial managers to provide relevant safety and insurance measures including Public Liability Insurance and appropriate Risk Assessment to protect participants and members of the public if applicable.

SWUK cannot directly be held responsible for the conduct of the trial manager, staff and volunteers or loss of possessions or personal injury to any dog/handler team in attendance at a SWUK trial, venue or local area.
